Management escalation pathway
Withhold ICPI treatment until normalisation to grade I
Follow up
- Re-check blood sample every 3-5 days
- If there is no improvement after 7 days, consider consultation with a hepatologist, liver biopsy and initiating systemic corticosteroid therapy with Medrol 0.5 mg/kg once daily
- Consider rechallenge if normalisation to grade 1 or corticosteroids <10mg/day
- If there is no improvement after 7 days, consider consultation with a hepatologist, liver biopsy and initiating systemic corticosteroid therapy with Medrol 0.5 mg/kg once daily
- Consider rechallenge if normalisation to grade 1 or corticosteroids <10mg/day
Refractory cases
- If there is no improvement after 3–5 days, consider managing as a grade 3
